United Healthcare is seeking compassionate and dedicated Home Health Certified Nursing Assistants (CNAs) to provide personalized, high-quality care to private patients in their homes. This role focuses on delivering one-on-one patient-centered care, supporting daily living activities, and promoting comfort, dignity, and independence. As a Home Health CNA, you will play a vital role in improving patients quality of life while working closely with families and healthcare professionals.
Provide direct personal care to private patients in their homes, including bathing, grooming, dressing, toileting, and mobility assistance
Assist patients with activities of daily living (ADLs) and basic instrumental activities as outlined in care plans
Monitor and document vital signs, symptoms, and changes in patient condition
Support safe mobility, transfers, and positioning to prevent injury
Assist with meal preparation, feeding, and hydration as required
Maintain a clean, safe, and comfortable home environment for patients
Offer emotional support, companionship, and respectful communication
Follow infection control practices and safety protocols at all times
Communicate effectively with nurses, care coordinators, patients, and family members
Accurately complete care notes and reports in a timely manner
Valid Certified Nursing Assistant (CNA) certification
Knowledge of home health care practices and patient safety standards
Strong interpersonal and communication skills
Compassionate, patient-focused approach to care
Ability to work independently in a home-based setting
Basic computer skills for documentation and reporting
Physical ability to assist patients with mobility and personal care
Minimum of 1 year of experience as a CNA preferred
Prior experience in home health, private duty care, or personal care assistance is an advantage
Experience working with elderly, chronically ill, or post-operative patients is desirable
Flexible scheduling options available
Full-time, part-time, and per-diem shifts
