Remote Nursing Jobs is looking for a dedicated and experienced UAS Assessment Nurse to join our growing team. The primary responsibility of this position is to conduct comprehensive assessments under the Uniform Assessment System (UAS) of New York to evaluate the healthcare needs and eligibility of clients for managed long-term care (MLTC) services.

Key Responsibilities:
- Conduct in-home and virtual assessments using the Uniform Assessment System (UAS-NY) platform.
- Accurately document patient conditions, clinical status, and service eligibility.
- Identify patients healthcare needs and recommend appropriate MLTC services.
- Coordinate with healthcare providers, case managers, and social workers to develop care plans.
- Ensure compliance with New York State Department of Health (DOH) guidelines.
- Provide follow-up support to clients to ensure appropriate services are being delivered.
- Attend regular meetings and participate in interdisciplinary case reviews.
- Manage and prioritize caseload effectively to meet deadlines and service goals.
Required Skills and Qualifications:
- Active Registered Nurse (RN) license in the State of New York.
- Proficiency with UAS-NY system for assessments.
- Excellent verbal and written communication skills.
- Ability to work independently with minimal supervision.
- Strong organizational and time-management skills.
- Tech-savvy with experience using telehealth tools and electronic health records (EHR).
- Compassionate and patient-focused mindset with excellent interpersonal skills.
- Detail-oriented with the ability to ensure accurate documentation.
Experience:
- Minimum of 2 years of experience as an RN, preferably in managed care or community health settings.
- Experience in conducting UAS-NY assessments is highly preferred.
- Previous exposure to remote nursing or hybrid roles is a plus.
Working Hours:
- Full-time position with Monday to Friday schedule.
- Hybrid model: In-person visits in Brooklyn combined with remote work.
- Flexibility to schedule assessments during patient-preferred times, when necessary.
Knowledge, Skills, and Abilities:
- In-depth knowledge of New York healthcare regulations and managed long-term care programs.
- Familiarity with MLTC eligibility criteria and assessment guidelines.
- Ability to adapt to changing workloads and manage priorities in a remote setting.
- Strong problem-solving skills with the ability to make clinical decisions independently.
