Key Responsibilities:

Required Skills and Qualifications:

  • Valid Registered Nurse (RN) license in the state of practice (multi-state license preferred).
  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience considered.
  • Strong understanding of care coordination, patient management, and health advocacy.
  • Certification in Case Management (CCM) or willingness to obtain within the first year of employment.
  • Proficiency with Electronic Health Records (EHR) and telehealth platforms.
  • Exceptional interpersonal, communication, and leadership skills.
  • Ability to work independently, make critical decisions, and handle sensitive patient information with discretion.
  • Knowledge of healthcare regulations and compliance requirements (HIPAA).
  • Strong problem-solving skills and ability to adapt to evolving patient needs.

Experience:

  • Minimum 3 years of clinical nursing experience, preferably in case management, home health, or chronic care.
  • Prior experience in remote nursing or telehealth is highly desirable.
  • Experience in managing multiple, complex cases simultaneously.

Working Hours:

  • Flexible working hours (Monday to Friday) with an 8-hour shift.
  • May require occasional availability for after-hours patient follow-ups or urgent virtual assessments.
  • Full-time remote position with the ability to work from home or any location with reliable internet access.

Knowledge, Skills, and Abilities:

  • Deep knowledge of case management principles and patient-centered care.
  • Ability to stay organized and effectively manage time with minimal supervision.
  • Familiarity with chronic disease management, post-acute care, and health coaching techniques.
  • Comfortable working in a digital environment, utilizing software, video conferencing, and online tools.
