Aetna is seeking a dedicated Telehealth Nurse Case Manager to join our innovative healthcare team. Key Responsibilities:
- Conduct thorough assessments of patient health and social needs via telephonic interactions.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
- Monitor patient progress, adjust care plans as necessary, and provide education on disease management.
- Facilitate access to healthcare services, including specialists, home health care, and community resources.
- Advocate for patients needs and support them in overcoming barriers to care.
- Document all interactions, assessments, and care plans in accordance with company policies and regulatory requirements.
- Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.
Required Skills and Qualifications:
- Current and unrestricted RN license in the state of practice.
- Bachelor degree in Nursing (BSN) preferred; equivalent experience may be considered.
- Excellent communication skills, both verbal and written.
- Strong organizational and time management abilities.
- Proficiency in using telehealth technology and electronic health record systems.
- Ability to work independently and make sound clinical decisions.
Experience:
- Minimum of 3 years of clinical nursing experience required.
- Previous case management or telehealth experience preferred.
- Experience in managing chronic illnesses and coordinating care for complex patients.
Working Hours:
- Full-time position with flexible working hours.
- Occasional weekends or holidays may be required based on patient needs.
