Key Responsibilities
- Conduct comprehensive health assessments and physical examinations for patients.
- Diagnose and treat acute and chronic illnesses, ensuring evidence-based practice.
- Develop, implement, and evaluate individualized care plans in collaboration with patients and families.
- Provide patient education on disease prevention, health promotion, and self-management strategies.
- Prescribe medications and therapies as necessary, adhering to relevant regulations.
- Collaborate with interdisciplinary healthcare teams to coordinate patient care.
- Maintain accurate and timely documentation of patient interactions and treatments.

Required Skills and Qualifications
- Master degree in Nursing from an accredited program.
- Current Nurse Practitioner license in Ontario.
- Certification in the relevant specialty area (e.g., Family Nurse Practitioner, Adult-Gerontology).
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Proficiency in electronic health records (EHR) and telehealth platforms.
- Ability to work independently and as part of a collaborative team.
Experience
- A minimum of 2 years of clinical experience as a Nurse Practitioner.
- Experience in a primary care or community health setting is preferred.
- Familiarity with telehealth services and virtual patient care models is an asset.
