Key Responsibilities

• Conduct comprehensive health assessments and physical examinations for patients.

• Provide patient education on disease prevention, health promotion, and self-management strategies.
• Prescribe medications and therapies as necessary, adhering to relevant regulations.
• Collaborate with interdisciplinary healthcare teams to coordinate patient care.
• Maintain accurate and timely documentation of patient interactions and treatments.
• Participate in quality improvement initiatives and continuing education opportunities.

Required Skills and Qualifications

• Master degree in Nursing from an accredited program.
• Current Nurse Practitioner license in Ontario.
• Certification in the relevant specialty area (e.g., Family Nurse Practitioner, Adult-Gerontology).
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills.
• Proficiency in electronic health records (EHR) and telehealth platforms.
• Ability to work independently and as part of a collaborative team.

Experience

• A minimum of 2 years of clinical experience as a Nurse Practitioner.
• Experience in a primary care or community health setting is preferred.
• Familiarity with telehealth services and virtual patient care models is an asset.
