Key Responsibilities:

  2. Patient Advocacy: Serve as an advocate for our Medicare beneficiaries, ensuring they receive the appropriate care and support throughout their healthcare journey.

  3. Data Analysis: Analyze healthcare data to identify trends, gaps in care, and opportunities for improvement. Utilize data-driven insights to make informed decisions.

  4. Collaboration: Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to enhance the coordination of care and improve patient outcomes.

Required Skills:

  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN): You must hold a valid LPN or LVN license in the state of practice.
