GracePath Healthcare is seeking compassionate and skilled Dementia Care Specialists to provide person-centered, high-quality care to patients with Alzheimers and other forms of dementia across our U.S.-based facilities. This role is ideal for experienced caregivers, nurses, or healthcare professionals who are passionate about improving the quality of life for individuals with memory-related conditions. Deliver direct care to patients living with dementia, ensuring safety, dignity, and comfort at all times.
Develop and implement individualized care plans in collaboration with physicians, nurses, and families.
Monitor behavioral patterns and cognitive changes, reporting concerns to clinical staff.
Assist with daily living activities such as bathing, dressing, mobility, and feeding.
Use memory-support techniques and therapeutic activities to stimulate cognition.
Maintain accurate documentation and patient records in compliance with HIPAA.
Educate and support families on disease progression and effective communication techniques.
Participate in care team meetings and contribute to quality improvement efforts.
Certified Nursing Assistant (CNA), Registered Nurse (RN), or equivalent caregiving certification (preferred but not mandatory).
Strong interpersonal and communication skills, especially with elderly patients.
Demonstrated understanding of dementia-related conditions and caregiving techniques.
Patience, empathy, and emotional intelligence.
Basic proficiency in English (spoken and written).
Minimum of 1–2 years of professional experience caring for individuals with dementia, Alzheimers disease, or cognitive impairments in clinical, residential, or home care settings.

Full-Time: 40 hours per week
Rotational shifts, including weekends and holidays (as required)
Overtime opportunities may be available with additional compensation
In-depth knowledge of dementia-related diseases and progression
Understanding of safety protocols, infection control, and elder care ethics
Ability to respond effectively in challenging or emotional situations
Familiarity with care documentation systems and electronic health records
