Job Summary
Aetna Health Services is seeking a compassionate and detail-oriented Remote RN Case Manager to join our dynamic healthcare team. Key Responsibilities
- Conduct comprehensive assessments of patients’ medical, social, and psychological needs.
- Develop, implement, and monitor individualized care plans in collaboration with patients, families, and healthcare providers.
- Coordinate with multidisciplinary teams including physicians, specialists, and social workers to optimize patient outcomes.
- Provide education, counseling, and support to patients and caregivers on treatment plans, disease management, and available resources.
- Monitor patient progress and adjust care plans as necessary to meet evolving healthcare needs.
- Document patient interactions, care plans, and outcomes accurately in compliance with regulatory and company standards.
- Ensure adherence to Aetna’s quality standards, policies, and protocols.
- Serve as a resource for healthcare professionals, providing guidance on care management best practices.
Required Skills and Qualifications
- Active and unrestricted Registered Nurse (RN) license in the applicable state(s).
- Proven experience in case management, care coordination, or utilization review preferred.
- Strong clinical knowledge across multiple healthcare settings and conditions.
- Excellent verbal and written communication skills to effectively interact with patients, families, and healthcare teams.
- Proficiency with electronic medical records (EMR) systems and telehealth technology.
- Ability to manage multiple cases simultaneously and prioritize tasks effectively.
- Strong problem-solving, critical thinking, and organizational skills.
Experience
- Minimum 2–3 years of nursing experience, preferably in case management, managed care, or chronic care management.
- Prior experience in remote healthcare or telehealth settings is a plus.
Working Hours
- This is a full-time remote position with standard business hours.
- Some flexibility may be required to accommodate patient schedules and time zone differences.

- In-depth understanding of clinical guidelines, care management principles, and disease management programs.
- Ability to educate and guide patients on healthcare decisions, treatments, and lifestyle modifications.
